Cervical cancer virus fuels oral cancer type, too

WASHINGTON (AP) — A prolonged sore throat once was considered a cancer worry mainly for smokers and drinkers. Today there's another risk: A sexually transmitted virus is fueling a rise in oral cancer.

The HPV virus is best known for causing cervical cancer. But it can cause cancer in the upper throat, too, and a new study says HPV-positive tumors now account for a majority of these cases of what is called oropharyngeal cancer.

If that trend continues, that type of oral cancer will become the nation's main HPV-related cancer within the decade, surpassing cervical cancer, researchers from Ohio State University and the National Cancer Institute report Monday.

"There is an urgency to try to figure out how to prevent this," says Dr. Amy Chen of the American Cancer Society and Emory University, who wasn't part of the new research.

While women sometimes get oral cancer caused by the HPV, the risk is greatest and rising among men, researchers reported in the Journal of Clinical Oncology. No one knows why, but it begs the question of whether the vaccine given to girls and young women to protect against cervical cancer also might protect against oral HPV.

HPV vaccination is approved for boys to prevent genital warts and anal cancer, additional problems caused by human papillomavirus. But protection against oral HPV hasn't been studied in either gender, says Dr. Maura Gillison, a head-and-neck cancer specialist at Ohio State and senior author of the new research. That's important, because it's possible to have HPV in one part of the body but not the other, she says.

A spokeswoman for Merck & Co., maker of the HPV vaccine Gardasil, said the company has no plans for an oral cancer study.

Monday's research was funded by the NCI and Ohio State. Gillison has been a consultant to Merck.

There are nearly 10,000 new cases of oropharyngeal cancer a year, and overall incidence has risen by 28 percent since 1988 even as other types of head-and-neck cancer have been declining.

Tobacco and alcohol have long been the main causes of these tumors, which occur in the tonsils, base of the tongue and upper throat. But over the past few years, studies have shown HPV is playing a role in that rise, probably due to an increase in oral sex even as tobacco use was dropping.

The new study took a closer look, tracking HPV over time by directly testing tumor tissue from 271 patients that had been stored in cancer registries in Hawaii, Iowa and Los Angeles. The proportion that were HPV-positive rose from just 16 percent in the late 1980s to nearly 73 percent by the early 2000s.

Translate that to the overall population, and the researchers concluded that incidence rates of the HPV-positive tumors more than tripled while HPV-negative tumors dropped by half.

Oral cancer has always been a bigger threat to men than women. Gillison says women account for only about 1 in 4 cases, and their incidence is holding steady while men's is rising. That raises questions about gender differences in sexual behavior or whether oral HPV infection is likely to linger longer in men.

While HPV is the most common sexually transmitted infection, studies show women's bodies usually clear the virus from the cervix quickly; only an infection that persists for years is a cancer risk. It's not known if oral HPV acts similarly or even is as common.

Nor is it clear if oral sex is the only way it's transmitted, cautions Dr. Gregory Masters of the American Society for Clinical Oncology, an oncologist at Delaware's Helen Graham Cancer Center.

Regardless, just over 11,000 cases of cervical cancer will be diagnosed this year, a number that has been dropping steadily thanks to better Pap smears. (It's too soon to know what difference vaccination will make.) Gillison's team calculated that annual cases of cervical cancer will drop to 7,700 by 2020 — compared with about 8,700 cases of HPV-positive oropharyngeal cancer by then, about 7,400 of them in men.

The cancer society's Chen urged caution about those numbers, saying more data is needed. But she says two things are clear: First, patients with HPV-linked oral tumors have better survival odds than those with other types of this cancer, possibly because they tend to be younger. Studies are beginning to test if they can scale back today's treatment and thus suffer fewer long-term side effects such as problems with speech and swallowing.

And "just because you're not a smoker or drinker doesn't mean you can't get throat cancer," Chen says — so get checked for symptoms like a throat that's sore for longer than two weeks.

NEW YORK (Reuters Health) - Cancer of the back of the mouth and throat is on the rise, primarily because of more cases stemming from a viral infection called human papillomavirus (HPV), researchers report in a new study.

The number of people who were diagnosed with HPV-related oral cancer in 2004 was triple the number diagnosed in 1988, due largely, researchers suspect, to changes in sexual behavior that have helped spread the virus.

HPV is a very common sexually transmitted infection that can cause genital warts and certain cancers, including of the cervix, anus and penis.

Dr. Maura Gillison at the Ohio State University and her colleagues examined oral cancer tissue collected from 271 patients over a 20-year period.

The type of cancer they examined, called oropharyngeal, originates in the back of the tongue, the soft part of the roof of the mouth, the tonsils, or the sides of the throat.

They checked the samples for evidence of HPV infection and found that the HPV-related cases became more and more common each decade, while those samples that didn't test positive for the virus became less common.

From these results, the researchers estimate that HPV-related oral cancers now afflict 26 out of every million people in the United States, compared to eight out of every million people in 1988.

Dr. Tina Dalianis, a professor at the Karolinska Institute in Sweden who did not participate in this research, said she believes the increase in oral cancers "is due to an HPV epidemic."

"We believe that sexual habits have changed, and that there is an increase in sexual activity earlier on in life, with an exchange of many more sex partners in general," she wrote in an email to Reuters Health.

The current study, published in the Journal of Clinical Oncology, confirms what Dalianis had found previously in Sweden, that HPV-related oral cancers were becoming the dominant form of the disease.

Previously, tobacco had been the primary cause of oral cancer, and most oral cancer cases were HPV-negative.

Gillison's group found that HPV-negative cancers have been cut in half since the 1980s.

HPV-positive cases -- which had made up just 16 percent of oral cancer cases in the 1980s -- comprised more than 70 percent in the 2000s.

"This whole relationship between HPV-related head and neck cancer completely changes our ideas of who is at risk, how to treat the cancer, the prognostics of the cancer, and prevention," Gillison told Reuters Health.

The good news is that people diagnosed with the HPV-positive form of the cancer have a better prognosis, she said, and the cancer is more responsive to treatment.

There is also a potential opportunity for prevention using an HPV vaccine that is approved to prevent anal and cervical cancer, Gillison said.

If the vaccine can prevent these HPV-caused cancers, perhaps it can work for oropharyngeal cancers too, she added.

Gillison and her colleagues have received research funds or consulting jobs through the companies that sell HPV vaccines, and one of the companies is a financial contributor to an organization that helped to fund this study.

The vaccines are not proven to prevent oral cancer, and Gillison said it will be important to test whether they are effective for this increasingly common form.

Preschools shut as virus outbreak rages in Vietnam

HANOI, Vietnam (AP) — More than a dozen kindergartens in Vietnam have closed to deal with an outbreak of hand, foot and mouth disease that has killed 111 children and sickened more than 57,000 this year, an official and the government said.

Ten kindergartens in Hau Giang province were shut for 10 days to clean and check the students' health, said Nguyen Van Muoi, head of the provincial preventive medicine center. Four schools reopened last week, he said Monday.

The southern province has had 361 cases since June, and some 50 children are hospitalized. He blamed the increase on the virus' spread after children returned to school from summer break. Kindergartens in Vietnam are similar to preschools, accepting children ages 1 to 6.

"I hope with this measure (closing the preschools), the new cases will drop," Muoi said.

At least three more kindergartens in the southern Bac Lieu province and the central highland province of Dak Nong also have been closed, Monday's Vietnam News reported.

The Health Ministry says more than 2,000 new cases of hand, foot and mouth disease are being reported each week. In a typical year, the virus infects up to 15,000 children in Vietnam and kills 20 to 30 of them.

The Ministry has recorded 57,055 cases and 111 deaths in the country as of Sept. 22, including a 3-year-old child who died on Sept. 20 in Hanoi, its website said Tuesday. A more severe strain called enterovirus 71, or EV-71, was identified earlier in about a third of sampled cases. It can result in paralysis, brain swelling and death.

Southern provinces report 70 percent of the cases and 90 percent of the deaths, it said.

The World Health Organization has said three-quarters of the deaths have been children younger than 3 years old.

The virus is named from its telltale symptoms, including rash, mouth sores and blisters on the hands and feet. It is spread by sneezing, coughing and contact with fluid from blisters or infected feces.

No vaccine exists, but the illness is typically mild and most children recover quickly. The disease is caused by enteroviruses in the same family as polio.

They found they were better able to infect species closely related to their typical target species than species that were distantly related.

However, the research also suggested that when diseases make a big leap they may then spread easily in species closely related to the new victim, regardless of how closely related these are to the original target species.

Dr Ben Longdon, of Edinburgh University's school of biological sciences, who led the study, said: "Emerging diseases such as Sars, HIV and some types of flu have all got into humans from other species.

"Understanding how diseases jump between different species is essential if we want to predict the appearance of new diseases in the future."

More susceptible

By infecting more than 50 species of flies with three different viruses, the researchers showed that species closely related to a virus's usual target species were more susceptible than distantly related flies.

They also showed that groups of flies that were closely related were similarly susceptible to the same viruses.

The study, funded by the Biotechnology and Biological Sciences Research Council, Natural Environment Research Council, the Wellcome Trust and the Royal Society, was published in the journal PLoS Pathogens.

Scientists who first linked chronic fatigue syndrome, also known as ME, to a virus have withdrawn some of their findings.

They have said some of their findings were based on "contaminated data".

Meanwhile, a study in Science claimed the virus could not be reliably detected in ME patients, even in the labs which originally made the link.

Understanding of chronic fatigue syndrome is poor. It may be many diseases and the causes are uncertain.

There was a sense of hope for many patients when a study published in Science in 2009 showed that DNA from a mouse virus, XMRV, was present in 67% of patients with the illness, but only 4% of the general population.

Yet other scientists around the world could not find evidence of the virus. Many researchers began to argue that the most likely explanation was contamination of the laboratory samples.

It led to Science asking the authors to withdraw their findings and it published an editorial "expression of concern" saying that the validity of the study was "seriously in question".

One of the labs involved, the Whittemore Peterson Institute in Nevada, is standing by the conclusion. One of its lead researchers, Dr Vincent Lombardi, said it was "participating" in the retraction but: "We want to make it very clear that we are continuing the important work of studying retroviruses in association with ME/CFS and other similarly complex illnesses.

"WPI's more recent retroviral work, although still in the early stages of discovery, continues to warrant additional investigations."

Meaningless

Dr Jonathan Stoye, virologist at the Medical Research Council National Institute of Medical Research in the UK, said: "I don't think this partial retraction has any meaning, it would have been nice to have a complete rather than a partial retraction.

"They're saying the rest of the paper still stands, but that is becoming increasingly difficult for them to maintain."

A fresh study on XMRV published in Science, which the researchers behind the original study participated in, again questioned the link.

Samples of blood were collected from 15 patients who had previously tested positive for XMRV, 14 of whom also had chronic fatigue syndrome, and from 15 patients without XMRV.

These samples were sent to nine laboratories, including two which had found the link previously. No lab knew which samples were from which patients.

Only two laboratories, the two which initially proposed the link, detected any cases of XMRV. However, the virus was detected at "similar rates" in both groups of patients, the study said.

The results from the two laboratories were also "inconsistent" even when testing blood from the same patient.

Dr Charles Shepherd, medical adviser to the ME Association said: "These are very emphatic negative or inconsistent findings from the Blood Working Group study, along with the retraction of some of the original supporting data that was published in Science.

"So it is now looking extremely unlikely that XMRV is either linked to ME/CFS or that it has a disease-causing role.

"Having had their hopes raised that a treatable component to ME/CFS had been identified, it's not surprising that people are becoming increasingly disappointed at the way things are turning out.

But it's too early to send out the scientific jury to make a final definitive decision on XMRV and ME/CFS - we still need the results from the other major multi-centre study on XMRV and ME/CFS being carried out in America by Prof Ian Lipkin."

WASHINGTON (AP) — There is more evidence that a virus once thought to be linked to chronic fatigue syndrome was a false alarm. A U.S. study released Thursday concluded lab tests used to make that link are unreliable.

Also Thursday, the journal Science said researchers were withdrawing part of the original study that suggested the connection — although not its conclusions — because a laboratory that contributed to the work discovered contamination in some of its samples.

In 2009, researchers in the state of Nevada announced they had found a mouse-related virus named XMRV in the blood of chronic fatigue patients, fueling hopes that a cause of the mysterious illness might have been found. Blood banks began turning away donations from people diagnosed with chronic fatigue. Yet numerous other studies failed to confirm the findings. Last spring, Science declared any link "seriously in question" as genetic tracing suggested it was the result of laboratory contamination.

The newest study, also reported in Science, was part of government efforts to see if XMRV or related viruses might affect the safety of the blood supply. It concluded there's no reason to worry.

Nine laboratories re-tested blood samples from 30 people, some chronic fatigue patients previously reported to have XMRV and some healthy people known to be virus-free. Only the two labs that previously reported an XMRV link could find any signs of the virus in some samples — but some of those were from healthy people, and additional testing couldn't confirm the findings.

"There does not seem to be any evidence of an association of XMRV with chronic fatigue syndrome," said Dr. Harvey Klein, who was not part of the new research but monitors it for the international blood-banking association AABB. Still, blood banks may continue turning away chronic fatigue patients out of concern about their own health after losing blood, he added.

The controversy is not over. In a statement, researchers at Nevada's Whittemore Peterson Institute said, "We want to make it very clear that we are continuing the important work" of studying the virus, despite the partial retraction of their 2009 study.
